The symptomatic approach to treating AD involves maintaining or restoring the neuronal pathways that become deficient during the course of the disease. In early AD, a prominent deficit occurs in cholinergic pathways, particularly in the basal forebrain cholinergic system, whose afferents project widely to the hippocampus, amygdala and neocortical areas. The Cholinergic Hypothesis for AD held that the deficiency of presyn-aptic neurotransmitter was a central etiologic factor. The drugs currently on the market for treatment of AD are designed to boost presynaptic levels of acetylcholine, usually by inhibition of the breakdown of acetylcholine by cholinesterases. These cholinesterase inhibitors were developed through the use of mice or rats whose cholinergic function had been impaired by genetics (hippocampally-deficient mice), reversal of acetylcholiner-gic antagonist drugs, or lesioning. The animals were then assessed for behavioral improvement in the presence of the test agent. The compounds developed had the expected pharmacological properties, but they do not alter the course of the disease in a major way.
