The Glioma Outcome project or GO was a physician directed project with the goal of improving the care of brain tumor patients. A total of 788 HGG patients were enrolled. The project collected data on the use of alternative therapies by glioma patients. An abstract of this data was presented at the Society for Neurooncology meeting in 2000 (61), which evaluated data from the first 520 patients. Patients were asked about alternative therapy use within 3 wk of glioma surgery and at 3-mo intervals after that. Forty-nine percent of patients used at least one alternative therapy during their treatment. The most frequently used treatments included meditation and prayer (28 ), high-dose vitamins (23 ), and herbs (18 ). Interestingly, alternative therapy use at some point (49 ) was greater than the use of chemotherapy (25 ), or those who enrolled in clinical trials (21 ). An earlier study by Verhoef et al. These givens can live an authentic existence (Heidegger, 1962), true to their experience of the world. However, attempts to avoid the angst, or existential anxiety, which accompanies the awareness of one's essential nothingness (Kierkegaard, 1954) can lead a person into a life of inauthenticity or bad faith (Sartre, 1951), in which, for example, their actions are determined by conformity with values other than their own. Another path to psychological disturbance, but in this case to a likely diagnostic label of psychosis, is an authenticity untrammelled by any acknowledgement of external reality.

Attitudes and Anxieties

Related to, but more diffuse or generalized than fears are attitudes and anxieties concerning death. Many factors contribute to death anxiety, including the physical suffering that death may entail, being reduced to a state of nothingness or nonbeing, punishment after death for our wrongdoings, and the loss of all that we hold near and dear in life. Fatigue is prevalent and disturbing, but largely an ignored symptom because it is not life threatening (23). Fatigue can cause severe disruption in the patient's ability to function and also affect multiple aspects of life. Fatigue can be mild to debilitating and is usually cum-mulative. Fatigue is not predictable by tumor type, treatment, or stage of illness. High levels of fatigue impact on all aspects and correlate with impaired QOL (24). Fatigue may be one of the significant factors that limit a patient's QOL. Interventions include energy conservation, maintaining normal sleep-wake cycles, learning not to fight the fatigue, flexibility, preplanning, and setting limits and priorities to balance and organizing activities.
